During our lives most of us are extremely independent; very seldom are we unable to cope.  We are subject to change and most of the time we are able to cope and understand the changes we are going through. There are several major changes that we all experience at some stage in our lives, from childhood to retirement.  There are some of us who suffer a very difficult challenge of a physical infirmity and are in need of special help.
Top Notch Healthcare Services Role
The objective is to support and assist the Service user to sustain and if possible improve independence in their home.
 
It is the aim of Top Notch Healthcare Services to offer high quality care to ensure this can be achieved.
 
Our carers are familiar with, and able to care for, those Service users who are experiencing problems associated with old age and physical infirmity.  They are there to offer a range of services including personal care, housekeeping, domestic assistance and companionship.
 
We understand that whilst relatives and friends feel responsible and want to help, they are often unable to do so, or may just need a little assistance to ease the load.  Our carers are there to work with the Service user's family and friends, and are also there to work with the district nurse and other professionals who are involved in the care of your loved ones.              
Standards of Care
We believe that professional carers who have received professional training can only give high levels of care.  Our carers are all interviewed and carefully screened before we employ them.  We take the opportunity to obtain personal references, and also try to give the Service user the choice of standard of care he or she will receive.
Statement of Purpose
Following an Assessment, the service is available to the following clients:

Older adults. Adults who are unwell. Adults with physical disabilities, and those requiring palliative care.

We aim to provide you with personal care and support in order to help you remain living independently in your own home for as long as you wish and to promote the Service users independence.
 
Professional - trained carers provide a service in a manner that meets the needs of the Service User, as defined in their Individual Care Plans.
Service Delivery
From the initial referral, Top Notch Healthcare will commence to assess the service user's needs. This will be done with the service users wishes respected and taken into account.
 
The first step will be for an appropriate Top Notch Healthcare Manager to make an appointment to visit the new service user in their own home.
 
This is to enable a range of assessments to be carried out:
  • A  Moving & Handling Assessment
  • An Environmental Assessment
  • An Assessment of Needs
The second step will be to use the information gained in the Assessment process to develop the Care plan. This is a written document used to set out the health, Social care and support that a Service user needs. As well as the service user or other parties such as the G.P and/or Social Worker will also make a contribution to its development.
The Care Plan sets out the path to achieve the specific outcomes that will fulfil the needs of the Service user.
 
The third step once the Care Plan is completed to all parties' satisfaction will be to select a Care worker. Arrangements will then be made to introduce the Care Worker to the new Service user prior to the commencement of the Care Plan. 
 
 We understand that all our Service users will need regular reviews of their own personal care package and we aim to see that each Service user receives a review every three months, unless changes occur or is otherwise requested.  We will develop a personal care plan (with the Service user's agreement) and will expect our carers to keep a full record within that care plan of the day-to-day needs of the Service user.  This will enable us to respond to changes in the Service user's condition.
 
We take great care in choosing our carers for each Service user, seeking those with the right experience for the Service users' personal needs and, in particular, a temperament and background which will suit the Service users' expectations.
 
We insist that Service users are treated with the highest level of dignity and respect at all times and that all Service users' information is kept confidential at all times.
 
We understand that we will be working within the Service user's home, and as such, all carers should remember that they are part of the family situation whilst they are caring for the Service user, and we encourage our carers to undertake their duties to suit their families' needs.
 
We need to be certain that our carers are working and conducting their duties in a way to suit your needs, and we insist that should they have any concerns that they contact us straight away.  We also take this opportunity to do 'spot-check' visits to see that they are working and conducting themselves appropriately.  We are available 24 hours a day and will be more that happy to talk to our Service users should they ever experience any problems.

Community Care services
Our Community Care Service offers professional assistance for full or part-time care, holiday and convalescent care, live in care, day or night 24 hours a day.
 
Our Community Care Services are aimed at offering care for the young and the old, frail or infirm, those suffering from mental health or physical handicaps, those with learning difficulties, and the terminally ill.
Assistance with tasks our Care Workers can offer 
  • Assisting the Service user in & out of bed.
  • Personal Hygiene
  • With bathing-bed/bath room in bath or Shower
  • Assistance to dress/undress
  • Cooking of meals
  • Shopping
  • Light Housework
  • Prompt Medication
  • Companionship
  • Ironing
  • Assist to toilet/commode
  • Feed pets.

Some care workers are specially trained in intermittent catheterisation.

Tasks our Care Workers are NOT allowed to undertake:
  • Filling daily dose containers
  • Giving Suppositories
  • Giving Enemas
  • Giving Injections
  • Changing any dressings
  • Handling Syringe drivers
  • Performing Catheterization (carers are allowed to perform intermittetent self catheterisation)
  • Cutting Toe nails

Our Care Workers are not authorised to collect pensions without permission.

Medication Policy
Attention is drawn to our medication policy.

Our Care Workers are only authorised to prompt a service user to take their medication. On no account are they allowed to place medication in the mouth unless authorized to do so.

A copy of our medication policy is available upon request.
Training Room
We also have a training room. Please note that this is for hire, in addition to the care services we provide.

Included with the price (£80 per day)  is equipment such as Pens, Paper, Flip chart, Over head projecter, TV/DVD/VIDEO, Moving and Handling Equipment, and Beverages.
